Special Health Care Forms
Our district requires students to be current on immunizations against certain diseases. For exemptions based on medical or religious reasons, we can only accept official forms issued by the New Mexico Department of Health, Immunization Division. There is also a link to this exemption form below.
Sometimes students need to take medication at school. Please note that we do not allow students to carry medication of any kind on campus. If your child needs to take daily medication or uses an inhaler, you will need to provide these to the school nurse at your child's school for administration in the health office. Additionally, whether it is prescription medication or over-the-counter meds, special authorization forms are required. Some students also need to have special plans in place for providing the best possible care for certain health concerns or conditions. You should find many of these special authorization forms below. All health forms must be renewed each school year. If you do not find the forms you are looking for here, please call your school's health office for assistance.
